IMPORTANT:
You must bring two forms of original unexpired US Government Issued Identifications.
Driver's license or Passport +plus one other+
Second Form can be:
Social Security Card, Voter's Registration,
Birth Certificate, Conceal Carry License, EAD Card,
US Passport or International Passport with accompanying Visa page
You need to generate a Livescan Request Form.
Your Livescan form can serve as your 2nd form of ID.
Please bring your Social Security Card if your name differs on it
from your Livescan form or driver's license.

Audiologists will be receive a discount at the event.

INSTRUCTIONS TO GENERATE LIVESCAN REQUEST FORM
Go to the State operated Chai System to generate the required Fingerprint Livescan form:

* Sign in by receiving code in your email.
* Leave the CHAI page open or it will not accept code upon re-opening.
* If you are expired or do not have a profile, scroll down to the bottom and click "Initiate Screening".
*Update or enter your demographic data. Hit Next.
* Enter Any states you have lived in during the past 5 years. If none, just hit Next.
* Scroll in the list of careers to find the one that matches yours. Hit Next.
When you get to the privacy policy, you must acknowledge by clicking box in blue window. Sometimes it is hard to find. If you are on your cell phone, try turning your phone to landscape. * *
* Click Acknowledgement. Hit Next.
*IMPORTANT - The next screen is a live scan provider search. It is glitchy and takes you away from the State site. You don't need to select a vendor to get your form, BUT you need your form to get your fingerprints! Leave it blank - Ignore entering your City or County and just scroll down to "Continue without Making an Appointment". It is located just below the search criteria. This is not particularly remarkable and does not look like a hyperlink, but it will take you right to your Livescan form. Print it and take a screen shot. We must enter the your Screening ID in the fingerprint file, along with your ORI code.
